• Complete the survey for Operation Blessing
  • Operation Blessing contributes the majority of non-perishable food items in our self-select food pantry. Your feedback helps them continue to be a blessing to our pantry!
• READ thoroughly, and complete the back of the survey if you wish to partcipate in the Chance Donation Drawing.
• Turn this completed form in at one of the front desks to receive a ticket.
• Drop your ticket in the box representing the item you want a “chance” to win.
• Winners will be contacted by the phone number listed on your ticket.
